Directions to Access Your Patient Portal
To gain access to your patient portal, you will need to provide us with your email address. If you would like immediate access, please contact us and your portal can be set up within 1-2 business days. After confirming that your portal is ready with us, Click the Patient Portal Button. Once there, you will click forgot password. Enter your username (the email you provided to us), your date of birth (mm/dd/yyyy), and last name. You will be sent an email from Modernizing Medicine EMA. Follow the link in the email to verify your information and create your password. You can now access all your medical records and make changes in the portal. We hope this service will be an easy and convenient way for you to access your medical records. |
